Granulomatous hypophysitis.

چکیده
364 CMYK hyperintense bright spot signal of the normal neurohypophysis, thickening of pituitary stalk and enlargement of neurohypophysis in cases where it is involved. A review of literature revealed only two case reports of this association with lymphocytic hypophysitis (both in female patients), and one case with granulomatous hypophysitis (intrasellar tuberculoma). Surgical intervention is required to establish the diagnosis and to reduce the size of the lesion to relieve the mass effect on adjacent structures. Prasad et al. proposed a management paradigm for suspected cases of lymphocytic hypophysitis and advocated a trans sphenoidal stereotactic biopsy to achieve a tissue diagnosis, which might avoid the need for an open exploration of the sella. The present case highlights the fact that apoplectic changes may complicate the course of inflammatory pituitary lesions, thus calling for a greater vigil and clinical judgment in management of these patients.

Lymphocytic and granulomatous hypophysitis: experience with nine cases.
OBJECTIVE Lymphocytic hypophysitis and granulomatous hypophysitis are rarely encountered. The aim of this study was to demonstrate their clinical peculiarities among pituitary disorders and to provide an approach for their clinical management. METHODS In a retrospective study, we reviewed our surgical experience with nine patients harboring hypophysitis. The series included six cases of lymph...
متن کاملPrimary hypophysitis: clinical-pathological correlations.
OBJECTIVE Primary hypophysitis comprises of three distinct histomorphological entities: lymphocytic, granulomatous and xanthomatous. Clinical features of the three subtypes for diagnostic and treatment strategies have yet not been well characterized. METHODS Endocrine function, visual fields and acuity as well as magnetic resonance imaging characteristics were assessed before and after transp...
متن کاملGranulomatous hypophysitis due to Wegener's granulomatosis.
We describe the MR image findings in a case of granulomatous hypophysitis due to Wegner's granulomatosis. A high index suspicion of hypophysitis based on imaging findings allowed successful medical management and helped avoid surgery. The MR imaging features included a thickened stalk, a diffusely and uniformly enlarged gland, a normal size or minimally enlarged sella, and enhancement of the op...
متن کاملGranulomatous hypophysitis by Mycobacterium gordonae in a non HIV-infected patient
Lymphocytic or granulomatous hypophysitis is a rare entity with a difficult diagnosis. Our objective was to report a patient with non-tuberculous granulomatous hypophysitis. An HIV-negative 45-year old man with confusional state, subacute ophthalmoplegia, and clinical and laboratory findings of panhypopituitarism was seen in the emergency unit. A cranial MRI showed a sellar mass suggestive of h...
متن کاملGranulomatous hypophysitis: rare disease with challenging diagnosis
Granulomatous hypophysitis is rare pathology that mimics pituitary adenoma. Diagnosis is only confirmed by histopathology examination. Trans-sphenoidal surgery is considered diagnostic when descent tissue specimen is obtained and therapeutic by decompressing optic pathway and the sella. Pathological findings always reveal granulomatous areas, multinucleated giant cells, plasma cells, and lympho...
متن کاملSilent ACTH cell adenoma in coincidence with granulomatous hypophysitis--a case report.
A 44-year-old male suffered from an inactive pituitary adenoma measuring 20mm in computed tomography (CT) scan and nuclear magnetic resonance tomography (NMR). The tumor was surgically resected via the transnasal-transsphenoidal route. Surgical specimens revealed a typical, sparsely granulated adrenocorticotropic hormone (ACTH) cell adenoma, but also a granulomatous inflammation mostly in the p...
